You’re welcome to fill out your paperwork before your visit. Click the link above and print out the new patient packet. Please complete the first 4 pages and bring them with you to your appointment. 

We firmly believe that finances shouldn’t be an obstacle for achieving life-long benefits of healthy, clear vision. While the actual cost of eye care varies depending on the treatment required, our financial team will help develop a personalized payment plan that meets your needs. We provide a variety of flexible payment options to make your treatment affordable, and will thoroughly discuss these options before treatment begins.

Payment Options For the convenience of our patients, we accept Visa, MasterCard, American Express, Care Credit, cash, and checks.

Insurance We accept most health insurance policies. We do not accept vision insurance. Remember that insurance policies vary, and we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims. If, during the course of treatment, your benefits change, the financially responsible party is accountable for notifying us of all changes. Copays and coinsurance are due at time of service.
